How do I send in a traffic violation letter in Arizona?

Just got speeding ticket and I want to send it in quickly. I was given envelope and papers to include but I have no experience in this stuff. Does the envelope need a stamp on it or just give my name and etc... and also the brochure looking paper said to "detach and mail this page," so does that mean rip out that section of brochure with option I'm selecting? (does'nt look like it is meant to be ripped out which is why I'm asking)
